چکیده

Blood-group type matching has been well known as the principal rule in organ transplantation. However, as the possibility of obtaining grafts from living donors is very low, adjustment of blood type is sometimes very difficult. On the other hand, and particularly in Far East countries, it is even harder to obtain grafts from deceased donors; thus, living donors must be used for the rescue of patients with endstage liver disease. In Japan, this background has led to an increase of ABO-Incompatible (ABO-I) living donor transplantation, especially in the field of kidney transplantation. Although various therapeutic modalities emerged from the experience with renal transplantation, further adjusting strategies are required for the success of ABO-I liver transplantation [1]. Moreover, the hurdle posed by ABO-I liver transplantation is higher than that in kidney transplantation. This review describes the current advances of ABO-I liver transplantation including the achievements fruit of our predecessors’ efforts, and a proposal to improve outcomes and reduce ABO incompatibility related complications in liver transplantation.

Living Donor Liver Transplantation for Patients with Hepatocellular Carcinoma

BACKGROUND & AIMS Living-donor liver transplantation (LDLT) can simultaneously cure hepatocellular carcinoma (HCC) and underlying liver cirrhosis, improving long-term results in patients with HCC. ABO-incompatible LDLT could expand the living-donor pool, reduce waiting times for deceased-donor liver transplantation, and improve long-term survival for some patients with HCC. METHODS We retrosp...

First successful perinatal management of pregnancy after ABO-incompatible liver transplantation

Many papers have reported on pregnancy and delivery after liver transplantation, but there have been no reports on pregnancy after ABO-incompatible liver transplantation. This paper reports the first successful pregnancy and delivery of a newborn after ABO-incompatible liver transplantation for fulminant hepatic failure. The patient was a 39-year-old female. She had an ABO-incompatible liver tr...

Adult Living Donor Liver Transplantation Across ABO-Incompatibility

The objective of this study was to evaluate the results of adult ABO-incompatible living donor liver transplantation (LDLT).ABO-incompatible LDLT is an aggressive treatment that crosses the blood-typing barrier for saving lives from liver diseases. Although graft and patient survival have been improved recently by various treatments, the results of adult ABO-incompatible LDLT require further ev...

Experience with primary liver transplantation across ABO blood groups.

THE LIVER has long been regarded as a privileged organ which can be transplanted across incompatible ABO blood groups with little risk of hyperacute rejection. l However, in a recent review of 671 first, second, and third liver transplants we found a significant advantage for ABO donor-recipient identity for primary liver transplants.2 Although a large number of ABO mismatched grafts were succe...

ABO Incompatible Kidney Transplantation—Current Status and Uncertainties

In the past, ABO blood group incompatibility was considered an absolute contraindication for kidney transplantation. Progress in defined desensitization practice and immunologic understanding has allowed increasingly successful ABO incompatible transplantation during recent years. This paper focused on the history, disserted outcomes, desensitization modalities and protocols, posttransplant imm...
